Good Practice

What should be reported?

All Events must be reported, these include:
   - Personal Injuries (including RIDDOR reportable events)
   - Dangerous Occurrences (including RIDDOR reportable events)
   - Near Misses
   - Cargo Damage (including cars)
   - Plant/Asset Damage
   - Customer Complaint
   - Compliance Failure
   - Environmental Breaches
   - Supplier/Contractor Failure
   -Third Party Property / Asset damage
   - Marine related Events (including MAIB reportable events)

What key information should be included?

   - Photographs to capture the scene (can also be added to the Event in Ultimo).
   - Details of person(s) involved.
   - Statements from individuals involved (if possible).
   - Details of injuries and any treatment given.
   - Details of any vehicles or property involved.
   - Details of any Plant / Asset damage.
   - Details of any Third-Party involvement.
   - Details of any Third-Party damage.
   - Immediate mitigating actions that have been put in place.

Five Golden Rules of Reporting?
1. Never use people’s names in the reporter’s text.
2. Do not use unprofessional or inflammatory language.
3. Do not assign blame, guilt or liability.
4. Be clear, concise, and purely factual (i.e. avoid opinions).
5. Be as specific as possible.​​​

What information must an Event Report contain (mandatory requirements):

   - Title -> What has happened in short?
   - Description -> What has happened in more detail?
   - Date -> When did the Event happen?
   - Event Category -> Select from dropdown list:
         1. Accident (select this if unsure of below accident categories)
             1.1 Injury / Ill health (select if someone has been harmed and can also include asset damage)
             1.2 Damage Only (select if no one has been harmed but asset(s) have been damaged)
         2. Incident (select this if unsure whether event was a near miss)
             2.1 Near Miss (select if near miss)
         3. Observation (select this if unsure of below observation categories)
             3.1 Unsafe Condition (select if a physical safety undesired circumstance).
             3.2 Unsafe Act (select if a process / procedural safety undesired circumstance).
             3.3 MCA Detention (select if MCA audit a vessel and order a detention).
   - Site -> Select from Ultimo Site list.
   - Map Location -> Click on the map and select location.
   - Location Description -> Free-text location information as required.
   - Marine Event -> Yes / No (Marine Events require other mandatory information).
   - Weather Conditions -> Dropdown list of weather conditions.
   - Responsible Department -> Select from Ultimo department/section list.
   - Immediate Actions Taken -> Select from immediate actions list.

Other mandatory requirements will also be required if:
   - Personnel have been injured
   - Assets have been damaged
   - Environmental concerns have been raised.
   - Cargo has been damaged.
